Employee Decommissioning Request

Description of Service:

When an employee departs from the University of Wisconsin - Green Bay, IT services need to be decommissioned to ensure security and proper resource management. This service request allows departments to initiate the decommissioning process for employees, including deactivation of network accounts, email addresses, and access to systems.

NOTE: Supervisors must submit the request for employee decommissioning.

Request Guidelines:

To complete this service request, please provide all required information about the departing employee. This includes the anticipated end date, department details, and any specific decommissioning needs.

Procedure:

  1. Submit the completed Employee Decommissioning form.
  2. The GBIT Service Desk will review the request and proceed with deactivating the required services.
  3. Confirmation will be sent once the decommissioning is complete.
