Description of Service:
What is this form for?
This form is used to submit a project request to the GBIT team at UW-Green Bay. Project requests are larger-scale efforts that often require coordination, planning, and resource allocation. Examples include implementing a new system, migrating a service to the cloud, or planning a department-wide IT initiative.
NOTE: This is NOT the same as submitting an IT support ticket.
If you are requesting help with an existing system, need troubleshooting, or are requesting smaller enhancements (e.g., adding users, updating phones, small changes to a website), please visit the GBIT Service Desk instead.
What is a project?
A project is a temporary, planned effort to create a unique product, service, or result. Projects typically have:
- A defined scope
- A start and end date
- Resource needs (time, money, staff)
- Deliverables that provide value to the University
Common project types include:
- Implementing a new software or tool
- Rehosting or redesigning an entire website
- Large equipment rollouts
- Infrastructure upgrades or replacements
- System integrations or major changes to data workflows
What is not a project?
Requests that fall under regular day-to-day operational tasks—like password resets, workstation replacements, minor system updates, or modifying existing configurations—should be submitted as service desk tickets. These are not considered projects and submitting them here may delay resolution.
Request Guidelines:
Before submitting
Ensure you have a project sponsor (a leader or stakeholder who supports the initiative) and enough details to explain the scope, need, and potential benefits of the project.
What happens next?
After submitting this form, the request will be reviewed by GBIT’s Project Management team. It may be evaluated by IT Leadership and University administration for prioritization based on available resources and institutional priorities.

Procedure:
Submit your idea
Complete the form with as much detail as possible. Incomplete forms may delay review.
Review and evaluation
Project requests will be scored for impact and effort, then reviewed by GBIT leadership and university administration if needed.
Follow-up
You’ll be contacted with the outcome of your request, which may include approval, further discussion, deferral, or rejection.
