Use this form to report feedback or issues with AI agents created or maintained by GBIT.
This includes technical issues, response quality concerns, interface issues, error messages, or other feedback about how the agent is working.
What this form is for
- Reporting that a GBIT-created or GBIT-maintained AI agent is not working as expected.
- Reporting an error message, broken link, access issue, or interface issue.
- Sharing feedback about an agent response that seemed incorrect, incomplete, outdated, confusing, or unhelpful.
- Suggesting improvements to how a GBIT-maintained AI agent responds or guides users.
What this form is not for
This form is not for general help with the topic the agent supports. For example, if an agent supports purchasing and you have a purchasing policy question, contact the department or team that owns the purchasing process.
This form is also not for personal AI agents or agents that are not created or maintained by GBIT.
Before you submit
- Include the name of the agent.
- Describe what led up to the issue.
- Include any error code or message, if one appeared.
- Attach a screenshot if it would help GBIT understand the issue.
- Do not include passwords, Social Security numbers, health information, student records, or other sensitive information.
What Happens Next
GBIT will review the submitted information and determine whether the issue can be resolved directly, needs additional troubleshooting, or should be shared with the department or team that owns the agent content.
